As varied as the genres of Manga are, there is also a wide variety of sexually explicit content available for readers who enjoy that sort of thing. There is a wide spectrum of genres within Manga's more explicit side, ranging from the more soft-core Ecchi (manga with sexual overtones) to more hardcore Hentai (which is as explicit as you can get).

This side of the industry is what gives Manga and Anime a bad name in the West. If you're an anime/manga fan and are open about your love of it, chances are at some point someone will ask you if you've ever read/watched a Hentai. And no matter your response, you'll probably be ridiculed for liking Manga even if the sort of things you read aren't explicit at all.
However in the East its the norm. Although some aspects of it are frowned up in Japan, the more sexually explicit content is more often than not just accepted by the general population. Often some aspects of fan service finds its way into more mainstream Manga genres as well, with a lot of Shōnen series featuring some form of fan service at some point. That doesn't change the fact that a lot of the topics and fetishes featured in sexually explicit Manga are just plain weird. But I guess that it wouldn't exist if there wasn't at least one person out there who the series was catering to the desires of.
